Section 1115 Reentry Services Demonstration

MDHHS is actively pursuing a demonstration under Section 1115 of the Social Security Act to support individuals as they transition from carceral settings back into community. The goal of the waiver is to improve care transitions, focusing on key health and health-related social needs to ensure improved continuity of care, reducing gaps in coverage and services upon release.

MDHHS will host two public hearings to provide an overview and discussion of the Section 1115 Reentry Services Demonstration, where all interested parties will have an opportunity to provide comments.
