Report fraud

If you witness, suspect, or know of fraudulent, improper, or suspicious activity involving any Michigan Department of State programs or documents, submit a tip including the following information:

• Your first, middle, and last name (including suffix, if applicable)
• Your phone number
• Your email address
• Your residential address
• Mailing address (if different)
• Relevant and detailed information about the fraudulent or suspicious activity:
  - Who committed the fraud? (Name, address, phone number, email address and the individual's description)
  - What did the individual do; what fraud is suspected?
  - Where did the fraud take place?
  - When did it happen?
  - How was the fraud committed?
  - Who else has knowledge of the fraud?

If your license is lost or was stolen, you can request to add an identity verification alert to your driving record to notify law enforcement that someone else may be using your license. The alert is not meant to prevent identity fraud or theft. However, it will signal law enforcement and Department of State staff to ask for sufficient proof of identity whenever your driver’s license number is run during a traffic stop or Secretary of State transaction.

Please note: It may take up to 30 days to post the alert to your record. Once added, it will remain on your driving record for seven years, unless you submit a request in writing to have it removed or extend the alert by submitting a new request form.

• Regulatory Monitoring Division - Investigates licensed and registered automotive-related businesses and other vehicle-related regulatory issues.
