Please Note: Online students are not required to comply with the University Immunizations Policy. In addition, medical students in the College of Medicine MD program, please do not upload your forms in DrexelOne; instead, please email your completed requirements for matriculation to imm_surv@drexelmed.edu

Immunization Forms

• 2024–2025 General Immunization Form [PDF] (This includes the Tuberculosis Screening Questionnaire)
• College of Nursing and Health Profession Students ONLY: Compliance form [PDF] (Behavioral Health Counseling, Health Services Administration, Health Sciences, and Nutrition and Foods students are not required to complete this form.)

Uploading Your Completed Immunization Record

• Review the deadline and policy information on Immunizations.
• Download the appropriate immunization form(s) from the list above.
• Go to DrexelOne.
• Find the Health and Wellness Resources channel on the Welcome page in the center column.
• Use the Medicat System to access the immunization portal.
• Log in using your DrexelOne credentials.
• Please review the instructions on the "Home" page.
  Use the "Immunizations" tab to access the immunization page to enter your immunization dates:
  • Use the "All Required" box and select each vaccine from the drop down list.
  • Enter the dates you received  your immunization.
  • Select the "Submit" button.
  • Return to the "Immunization" page to enter the dates for your next vaccine.
• Use the "Uploads" tab to upload your immunization records.
• Follow the instructions to upload your records.

Confirming Your Immunization Forms Are Compliant

Forms take 48 hours to process. You must revisit DrexelOne to make sure there are no issues with your form 48 hours after submitting your form online. If there is a problem with your form, you will need to visit the Medicat System for details on the missing information needed for compliance.

Compliant Forms

If the immunization upload channel disappears and there is a green checkmark next to Immunizations on your checklist, your form was accepted and you are compliant with this policy.

Non-Compliant Forms

You will receive a message within your Immunization channel instructing you to visit the Medicat System for details on the missing information. Once you have provided the missing information, be sure to check your immunization requirement status again within 24–48 hours.
