Cover Story (view full-size image): A difficult disease to diagnose, leptospirosis has a wide spectrum of clinical presentations, including life-threatening ones. Fresh water exposure coupled with skin injury or immersion can lead to disease, which puts flood victims, agricultural workers, military members, and recreational tourists at risk. Climate change should increase disease incidence, suggesting many people at risk would benefit from preventive measures such as pre- or post-exposure antibiotic chemoprophylaxis. Eight clinical trials and three meta-analyses have evaluated prophylaxis and failed to show a substantial benefit. Our review assessed strengths and weaknesses of previous trials and described flaws in trial design or conduct. Based on our analysis, we suggest how future trials may be better constructed to answer this important question. View this paper
