    The utilization of resistance ambulation exercises during the sports rehabilitation of the ACL athlete is popular during the rehabilitation process. The use of weighted sleds, rubber bands, etc., are frequently employed as a load (resistance) stimulus for the athlete to push and/or pull (towing) resulting in adaptation for enhanced strength qualities. These exercises, when appropriately prescribed, may also be incorporated to assist to address the presence of kinesiophobia, a common condition of concern for loading the ACL lower extremity for fear of increasing pain and/or reinjury. Kinesiophobia is a significant influence regarding the post-rehabilitation athlete’s inability to return to their previous level of athletic performance. Research has demonstrated that kinesiophobia is more prevalent in female vs. male athletes (Shah 2017) as well as lower kinesiophobia at 4 weeks post-surgery increased the odds of meeting advanced rehabilitation criteria at 12 weeks post-surgery (Chmielewski 2018). At the time kinesiophobia is documented or suspected, my empirical clinical experiences have accentuated to employ active tasks such as suitably weighted sled pushes (Figure 1A) and manually resisted rubber band “towing” (Figure 1B) resulting in the "intent of effort" which diverts the athlete’s focus from the fear of loading their ACL extremity to concentrate on successfully overcoming an applied resistance during full weight-bearing movement. An effective exercise performance requires the athlete to apply greater amounts of force into the ground surface area with their ACL extremity, resulting in the displacement of their body over a specific prescribed distance. The initial introduction of the rubber-band towing exercise vs. the weighted sled allows for the resistance to be altered manually throughout the exercise performance. These techniques have been successfully employed in the early stages (the initial 4 weeks) of the ACL sports rehabilitation process as well as during the athlete’s first day of attempted running. A recommendation is to have the athlete perform their initial attempt to run and observe their running gait. Employ the resisted rubber-band technique and upon exercise completion, instruct the athlete to repeat their running attempt. The athlete will likely demonstrate an improved (but not optimal) running gait.
