    The ACL athlete’s return to play (RTP) is a familiar discussion within the sports rehabilitation community. These conversations include, but are not limited to various testing methods, limb symmetry indexes, cognitive training, etc. All these factors are essential at various periods of the rehabilitation process, however, at the time the athlete is medically cleared for competition they must meet, if not exceed the physical quality abilities that existed prior to their ACL injury. To achieve this objective the athlete must endure the appropriate, and progressive application of high intensity exercise for adaptation to transpire. The RTP athlete is not only required to meet the physical demands of their sport of participation, but the physical abilities demonstrated by their peers whom they will compete at every practice as well as their opponent on the day of competition. Figures 1 and 2 present some of the physical quality standards for the sport of football at various levels of competition (Hoffman 2006). If these physical quality “standards of the game” are not realized, can the athlete successfully compete? Wouldn’t the risk of reinjury be a concern when the athlete is confronted by a much stronger, more powerful, and faster opponent? Although various programmed exercises with isokinetics, BFR, etc. may be ideal during the rehab process, isokinetic training velocities are too slow compared to sprinting velocities and BFR intensities are too low for producing sport strength standards. To achieve the physical quality standards of sport, an appropriate and progressive planned application of high intensity i.e. loads, velocities, etc. is required. There may be concerns with the application of high intensity during the rehabilitation process. Can the ACL reconstruction and soft tissue structures withstand such stresses? Two viewpoints are as follows, (1) Weren’t these anatomical structures able to tolerate these high stresses prior to injury? and (2) High stressors of 90% - 95% of the athlete’s physical ability is still submaximal. High stress exercise execution is safe when deemed appropriate because they are of submaximal effort. What is dangerous? The application of high stresses at inappropriate times and/or too often. The exercise application of what? when? how much? how often? is the talent of the rehabilitation professional. One of the many lessons I’ve learned from Hall of Fame NFL Head Coach Bill Parcells is that confidence is born of demonstrated ability. The demonstrated ability to successfully lift heavy loads, jump high and far, and run at high velocities will assist to restore an athlete’s mental belief in their ACL reconstructive surgery and abilities on the field. RTP requires the athlete to meet the physical quality standards of their sport of participation. A standard met by both their peers and opponents. To achieve this objective requires the application of high intensity stress during the sports rehabilitation process.

    Did you know that BFR training, when combined with high-intensity interval training (HIIT), can enhance physiological improvements in aerobic, muscular, and to some extent, anaerobic performance? This is quite fascinating because it suggests that BFR can provide an additive physiological training stimulus to HIIT protocols1. Moreover, a systematic review and meta-analysis comparing low-load BFR resistance training to conventional high-load resistance training in musculoskeletal disorders found that both methods are equally effective in inducing gains in muscle strength, muscle mass, and physical function2. This means that BFR training can be a game-changer for individuals who need to train at lower intensities due to injury or other limitations, but still want to achieve significant improvements in their fitness levels. So, BFR training is not just about the restriction—it’s about unlocking potential in a safe and effective way! 💪 Want Learn more?
