Social Work Services at the Mount Sinai Hospital

The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities.

Mount Sinai Health Partners (MSHP) is the management services organization developed to enhance the Health System’s implementation of population health initiatives, that is, programs of care that enhance the patient experience and health status, improve the health of patient populations and reduce utilization (e.g. reduction in unnecessary tests/labs being ordered, preventing Emergency Department visits and hospital admissions/readmissions) and healthcare costs. Population health initiatives particularly focus on preventive care. MSHP provides the infrastructure and machinery to support population health initiatives and includes services such as care coordination, information technology, workflow optimization, physician engagement and quality reporting. The Department of Social Work Services is positioned to take a lead role in the development of MSHP which will be providing transitional and care coordination services for a large group of individuals who are insured by a combination of, e.g., commercial insurers, Medicare Advantage Plans, Medicaid Managed Care Plans, employers, and unions. This position is based at Martha Stewart Center for Living (MSCL) and focuses on geriatric patients and their families.

Responsibilities

The MSCL triage social worker is an integral member of an interdisciplinary team and provides episodic care to all clinic patients identified with immediate social service needs. The worker conducts a brief biopsychosocial assessment of patients and their caregivers, and addresses reason for provider’s referral or patient’s self-referral, as well as issues identified during assessment. Reasons for referral include: mental health issues such as depression and dementia; patient safety; functional issues requiring homecare to assist with ADLs/IADLs; medication and medical regimen adherence; patient access to care including transportation; insurance/financial issues; caregiver/family stress; end of life issues; elder abuse among others. Types of supportive intervention include: face-to-face meetings; follow-up phone contact, supportive counseling; patient advocacy; referrals for community services; engagement with insurance providers; APS referrals; provision of resources; referral for ongoing case management.

Qualifications

• Exceptional skills in engaging patients/families and in assessment and coordination of resources.
• Exceptional organizational, collaborative and psychosocial assessment and intervention skills.
• Hospital experience and experience with geriatric patients preferred.
• Proficiency in MS Office: Word, Excel, Outlook. Ability to learn hospital ambulatory scheduling system, electronic medical record and specific care coordination applications.
• Bilingual (English/Spanish) preferred.
• Minimum one year in current position.

The Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $81999.92 - $94759.89 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
