Engagement Description
  • Staff will be responsible for calling member and engaging them in our care management program. Providing resources to help them with their mental health, socioeconomic stressors, physical health. Staff will be able to manage documentation in the Care Advance program once trained, and have knowledge of outlook, mobile apps, websites, Teams.
Top 3 Required Skills:
  • Licensed Social Worker provide evidence if received outside of the state of Michigan
  • Knowledge of working with the retired population or a great interest in working with the elderly
  • Quick learner of computer program and navigating within those programs
  • Social resource knowledge or where to find the resource knowledge
  • Mental Health Knowledge or worked in hospice
  • Worked on a team
  • Highly productive/ able to work be motived while working from home
Education/Certifications Include:
  • Licensed Masters Social Worker and able to practice in the state of Michigan
