Home Health Social Worker Assistant

Summary: The Home Health Social Worker Assistant provides support to the Home Health Social Worker in delivering social services to clients and their families. This role involves working closely with healthcare professionals, patients, and their families to ensure that the social and emotional needs of clients are met. The assistant helps in assessing patient needs, developing care plans, and providing resources and support.

Key Responsibilities

  • Assessing the social and emotional needs of clients.
  • Participate in the development and implementation of individualized care plans.
  • Provide counseling and support to clients and their families under the supervision of the Manager of Support Services.
  • Assist clients in accessing community resources and services.
  • Maintain accurate and timely documentation of services provided.
  • Collaborate with healthcare professionals to ensure comprehensive care for clients.
  • Conduct follow-up visits and check-ins to monitor client progress.
  • Participate in team meetings and contribute to case discussions.
  • Adhere to all policies, procedures, and regulations related to home health care and social work practices.

Resource Utilization

  • Coordinate travel time for visits and manage a caseload to deliver care in a cost-effective manner that demonstrates an understanding of home health reimbursement methodologies.
  • Assure proper maintenance of records per agency guidelines.
  • Maintain regular attendance
  • Function independently, but seeks and accepts guidance from supervisor, other members of the team and members of the community, as deemed necessary and appropriate.
  • Comply with state and federal guidelines and Agency policies and procedures.
  • Conduct all business activities in a professional, ethical and confidential manner.
  • Assure proper maintenance of records per agency guidelines.
  • Function independently, but seeks and accepts guidance from supervisor, other members of the team and members of the community, as deemed necessary and appropriate.
  • Comply with state and federal guidelines and Agency policies and procedures.
  • Conduct all business activities in a professional, ethical and confidential manner.

Secondary Duties

  • Perform speaking engagements on home care and provides in-service education to staff as required.
  • Attend meetings with community agencies for networking as assigned.
  • Mandatory participation in staff meetings, departmental meetings, team meetings and in-services.
  • Participate in committees and other related activities as required.
  • Perform other duties as assigned.

Qualifications

  • Has a baccalaureate degree in social work, psychology, sociology, or other field related to social work, and has had at least 1 year of social work experience in a health care setting:
  • 1-3 years supervised social work experience in a health care setting working directly with individuals preferred.
  • The social worker should be able to work effectively with a variety of disciplines in an individual and team setting and should have a working understanding of social systems along with the ability to implement pertinent state and federal regulations.
  • Must successfully complete a post offer, pre-employment criminal background check and physical / drug screen.
  • CPR certified.
  • Must be a licensed driver with an automobile in good working condition and have automobile insurance in accordance with PA laws.
