GENERAL SUMMARY OF DUTIES: The Director of Population Health is responsible for the success of Envoy’s essential functions including the clinically integrated network, accountable care organization, value-based contracts, and building of shared infrastructure. Envoy Integrated Health enables independent providers to collaborate meaningfully on quality improvement, patient safety, value-based care delivery and contracting, public health, advocacy, and community building. The Director of Population Health will plan, implement, and manage projects and initiatives to achieve the goals of the organization with a focus on public health, collaboration with State agencies and community partners, quality improvement, and population health management. This position will work closely with the Chief Medical Officer and Chief Executive Officer and may also work in tandem with and/or supervise the work of other staff.

ESSENTIAL FUNCTIONS:

  • Oversees success of the clinically integrated network with an emphasis on population health
  • Ensure the design, implementation and execution of quality improvement, care coordination, value-based care, and community-based programs.
  • Promote public health development initiatives through outreach to governmental and state agencies, community-based organizations, physician partners, and a broad range of other healthcare entities.
  • Serve as a partner to develop reportable metrics to demonstrate outcomes for the programs and initiatives.
  • Plan and implement a strategy for improving workflows and delivering actionable data to network members to achieve success in contracts and partnerships
  • Supports and upholds established policies, procedures, objectives, quality improvement, safety, environmental and infection control, and codes and requirements of accreditation and regulatory agencies.

Qualifications

QUALIFICATIONS

Education:

  • Preferred: Master’s degree with emphasis in business, public health or a healthcare related field
  • Minimum: Bachelor’s degree in business, public health or a healthcare related field

Experience:

  • Preferred: At least one year of management experience in healthcare setting
  • Minimum: Three years of administrative experience in a healthcare setting
