Responsibilities
Manage a caseload assigned by Care Management Leadership.
Conduct scheduled contacts with enrollees based on their needs.
Perform face-to-face visits as required, in homes, offices, or agreed locations.
Assess enrollees regularly to identify needs and address care gaps.
Complete care coordination activities promptly and in compliance with policies and standards.
Collaborate with Senior Care Manager to prioritize cases and mitigate risks.
Document enrollee interactions accurately and in a timely manner.
Participate in discharge planning and ensure smooth transitions of care.
Educate enrollees on health services and promote compliance with medical and dental care.
Apply advanced knowledge of care management to support assigned enrollees.
Coordinate follow-up care with vendors and providers.
Assist enrollees with transitions of care across various healthcare settings.
Enter authorizations for services as required by Care Management.
Qualifications
3-5 years of experience in managed care or care management.
Experience In Clinical Or Community Resource Settings Preferred.
Excellent verbal and written communication skills.
Bilingual in Spanish preferred.
Proficiency in medical terminology.
Familiarity with Microsoft Office suite and other computer applications.
Strong organizational and decision-making skills.
Ability to work under pressure, meet deadlines, and maintain exemplary customer service.
Participation in professional meetings and multidisciplinary collaborations.
Licensed in DC as a Social Worker (LICSW) or Registered Nurse (RN).
Case Manager certification preferred.
Associate's degree required; Bachelor's degree preferred.
Working Conditions
Moderate mental and visual stress, frequent PC use.
Some weekend, evening, or early hour availability required.
