Wyoming pharmacist interviewed by AP: “I’m working for free a lot…I love to serve the community. But I kind of resent having to do that because of large corporations, huge pharmacy benefit managers, that are making millions of dollars a year.” A powerful piece by Devi Shastri in the Associated Press highlights how devastatingly low reimbursements set by PBMs are crushing rural pharmacies. Pharmacies in rural areas fill crucial gaps in our health system, ensuring that many Americans don’t go without needed care. But with margins crushed so severely by greedy PBMs, staying in business and continuing to serve these communities is becoming a more and more tenuous existence for pharmacists in Wyoming and beyond. According to the article, some patients drive up to five hours to access care at this pharmacy that they cannot access anywhere else — and yet 25% of the prescriptions this pharmacy fills are reimbursed below cost. This is beyond unsustainable. This is why we say we need a real alternative to reliance on PBM reimbursements — an entirely new business model for the community pharmacy. Patients need this care. Pharmacists want to continue providing it. But PBMs are making that prospect more and more untenable. It's not that pharmacists can do it for cheaper, it's that doctors have to do it for more because of defensive medicine. If pharmacists were to enter the diagnosis game then their malpractice and defensive practices would be on par with any other diagnostician. However, where pharmacists can shine is prescribing medication for conditions that don't need much of a diagnostic hurdle. Preventing highly trained professionals such as PharmDs from prescribing is a major waste of resources. However, I have a feeling after a few back and forth with patients when they have to deny certain meds, many pharmacists will have preferred to not be in the prescribing game.

We’ve been saying for years that federal provider status for pharmacists is a great idea. This week, Forbes reported that a few familiar organizations have come to that conclusion as well — Amazon, Walmart, CVS and Walgreens. About time, guys! 😂 Jokes aside, this is a really big deal for pharmacists’ prescriptive authority. Notably, several of these organizations have already launched test-to-treat programs in their retail pharmacies and are adopting this stance after seeing the success of those programs. Another key factor here is that all of these organizations have massive lobbying capacity. If they see federal provider status as a worthwhile endeavor, they could potentially spend billions pushing it through the House and Senate through their lobbying arms. But that process could take quite a while — and most independent or regional-chain pharmacies don’t need to wait to bring advanced services to their pharmacies in almost every state. How can community pharmacies assist patients with social determinants of health? --- I recently read an article (in the comments) about utilizing #pharmacists for addressing #SocialDeterminantsOfHealth (SDoH). Pharmacists have always been in the position to help address SDoH: -Accessible with 89% of US patients living within 5 miles of a #pharmacy and no cost to talk to the pharmacist -Many more touch points with the patients compared to other healthcare professionals -Highly trusted The main issue was typically what to do with the information once we have it. --- The article describes two pilots run by RITE AID. Both involve asking SDoH questions to patients, but the second pilot provided the pharmacies local resources to share with patients that were experiencing SDoH barriers. Pharmacy employees in the second pilot felt like they could actually do something when the patient shared a personal issue. --- Any pharmacy could devote an hour once a year to searching online for local food, housing, or other SDoH assistance in their area. Even if you aren't explicitly asking patients about SDoH, those issues will come up and you'll be ready with a list of resources. That's a pretty good return on investment for helping patients. Mastering Pharmacy Billing and Prior Authorization As a professional with multiple back surgeries, a plethora of joint replacements, and 30 years of experience in healthcare revenue cycle support, today's topic regarding the significance of Prior Authorizations (PA) in Pharmacy Billing is especially important to me. Pharmacists play a critical role in the overall healthcare management of a patient. They serve as the first line of communication and consultation about medications. While their domain expertise is indeed commendable, the administrative support, particularly in Pharmacy Coding and Billing, should match their efficiency. The missing link here is often the correct and detailed implementation of PA. Prior Authorization is a requirement that your physician obtains approval from your health insurance plan to prescribe a specific medication for you. For your insurance to cover your medication, all the information submitted needs to be accurate, including the selection of the right ICD-10 diagnosis code, and not just “chest pain” or “asthma.” A claim can be denied due to the inconsistency between the authorized request and the actual claim submitted. Here's a common scenario - A drug was authorized to be dispensed once a day. But, when the claim was submitted, the frequency was changed to twice daily. Result? Denied Claim. Despite accuracy of diagnosis, the claim didn't match the authorized service due to a minor yet consequential error. If you find your healthcare provider continually struggling due to these petty issues, there's a way out. Hiring a certified coder or a professional remote staff can be beneficial in terms of efficiency and operational costs. Invest in regular chart audits and have a designated team for Prior Authorization. Train them to not easily accept a 'No' and encourage them for appeal attempts. Navigating through these billing challenges may be complex, But, remember, perseverance is the key to a smoother and less frustrating revenue cycle.
