Privia Health had a solid start to 2024 with continued growth and execution. One notable trend is Privia's pullback on capitation. Privia wants to be thoughtful about taking full risk - in markets and scenarios where it makes sense (and it's profitable of course). Flexibility is built into Privia's operating model, and you can see the levers being pulled in Q1 and in 2024 given significant MA headwinds. According to the latest report from Rock Health, digital health funding in Q1 2024 totaled $2.7B across 133 deals, with an average deal size of $20.6M. It doesn’t break out device-related investments, but most medtech founders I meet these days are being forced to chase tiny rounds made up of endless little checks, consuming valuable energy and slowing their progress. The benign neglect of this next generation of diagnostic and therapeutic devices is counterproductive and surprising. Given the crowded virtual care market, for example, I’d expect more such companies would be exploring medtech partnerships, investments or acquisitions that'd enable them to back integrate into delivering sharper, and more differentiated, care and treatment. This is exactly what Maverix Medical is doing from the other direction: Reshaping the entire spectrum of lung cancer care by integrating more advanced + effective approaches to screening and diagnosis, treatment and therapeutics, and even palliative care…And putting it all to work through an innovative provider network. One notable investor exception is Y Combinator partner Surbhi Sarna who’s been backing great teams like Selera Medical (YC W24) doing foundational work on ambitious medical devices. Without more support from investors like her, we’re going to increasingly find ourselves stuck with the incremental portfolio improvements from the incumbents, and that’s just not nearly enough.

So we appreciated this perspective from Justin R. Brueck at Endeavor Health, on three of the most common reasons he sees things go awry. https://lnkd.in/eKBZ_g2N    • We’re continuing to see stories illustrating both the promise and perils of leveraging generative AI in the practice of clinical medicine. Given the pace of progress, it seems clear that AI will play a significant role in clinical care sooner than later. Lots of thorny questions to work through in a short amount of time. One recent study published in JAMA found that AI scribes significantly reduced time spent in the EHR. Walmart is exiting healthcare. This doesn’t surprise me. Walmart was never designed for healthcare, it was designed for retail. Everyone wants a piece of the $4T pie, but it’s a pie full of indigestible ingredients that would make the biggest companies choke. Especially companies that were never built for healthcare. - fee-for-service doesn’t work, especially with very low fees - relying on legacy payors doesn’t work - using insurance for primary care doesn’t work - not paying providers enough doesn’t work - having a clinic next to gardening supplies doesn’t work Throwing millions at healthcare doesn’t automatically work with a large base that comes to you for something else. I believe a properly built and well-executed healthcare startup that “thinks differently”, will eventually move the needle on improving the American healthcare experience. It’s coming, and will surprise everyone.

Quarterly Q&As are out! These are mini refreshers or clarifiers for everyone in the home health industry! Question 1 is GOLD for businesses like ours. Question 1: A third-party external auditor consistently states that our OASIS functional status items are “underscored” and recommends changing the responses to the OASIS items. Is the assessing clinician required to accept the auditor’s recommendations? Answer 1: While a home health agency may use third party external auditors to review OASIS coding and make recommendations, the assessing clinician is responsible for determining OASIS coding based on their assessment. We often get clinicians who don’t understand the reasoning or rationale. The longer or more detailed it is— consider it unread. That is NOT compliance. Truth be told, most of the clinicians are reviewing these changes in the field or at home after a 10 hour home health shift. Their brains are at capacity for receiving more info. I should know, I was one of those clinicians. Family time is now met with 6 inch long “yellow sticky notes”. That’s why most of these clinicians have my personal email and phone number. I thrive on educating and so I always encourage them to “call me with questions”. You’ll usually find me at 9pm explaining why M2020 is likely marked as 3 if M1860 is stating patient requires assistance or supervision of another person at ALL times during ambulation. Want the inside story on what AI tools are being piloted at ScionHealth? This week I chatted with VP of Strategic Operations & Initiatives, Sarah Hughes, MSHA, MBA about how they’re using Eon, LUMINARE, and Operait Health to build out their AI toolkit. — 🟩 Q: What are your biggest tech-related priorities for 2024? 1) Workforce support—How can we deploy technology solutions that make our staff’s lives and daily work easier and more efficient? For example, we are currently deploying a technology platform, M7 Health, which has built a dynamic nursing workforce management system to help us understand and optimize each nurse’s work-life preferences with regard to scheduling and career goals. 2) Patient activation and engagement—How can we better engage and activate patients on both the front end (i.e., scheduling appointments and interacting with our organization) and the back end (i.e., continuing to engage with us to manage their health)? 🟩 Q: Where are you currently exploring AI-driven solutions? We’re running pilots to address a few different administrative and operational efficiency use cases: Incidental findings on radiology reads—We’re working with Eon to automatically scan radiology reads to identify and manage patients with incidental findings, and proactively reach out to them to schedule follow-up care. Sepsis early detection—We’re currently testing LUMINARE's platform, which implements the hospital’s sepsis protocols by automating intervention, streamlining communication, and simplifying reporting and analytics. Operating room scheduling—We’re also looking at a vendor called Operait Health which has built an AI-first solution to optimize surgical demand and right-size OR staffing by forecasting gaps in the OR schedule and placing cases unrestrained by blocks. 🟩 Q: Which areas are you most excited about the potential for new solutions? We’re excited about the potential for virtual remote care in the home on the long-term acute care (LTAC) side. We’ve already seen success with these models in our community hospitals, and we believe there’s potential to similarly deliver a better patient experience at lower cost by bringing care into the home for a subset of our LTAC patients. 🟩 Q: Any general advice for healthcare IT leaders out there? It can be easy to underestimate the legacy IT “tax” of bringing in any new vendor, given that any new solution must work with our existing tech stack. Vendors need to factor this in when discussing implementation timelines, and health systems need to ensure they include all the right internal stakeholders, including operational leaders, early in the conversation to appropriately plan for integrating and deploying solutions into day-to-day operations. — p.s. I loved this conversation because of all the practical wisdom Sarah shared. We're on the lookout for similarly insightful health IT leaders to feature. Who should talk to?

💼 Waystar's $950M IPO: A Game-Changer in the Health-Tech Sector? Waystar Holding Corp., a health-care payments software firm, is moving ahead with its US IPO, potentially launching as soon as Tuesday. The IPO could raise about $950 million through new shares, possibly valuing the company up to $6 billion. It seems to me that this IPO is actually the interesting one for several reasons. Firstly, the list of major investors like EQT, CPPIB, Bain Capital, and Francisco Partners already inspires confidence. Secondly, Waystar's revenue growth, from $705 million in 2022 to $791 million in 2023, despite net losses, clearly demonstrates robust demand for its services. Lastly, the involvement of top-tier underwriters like JPMorgan Chase, Goldman Sachs, and Barclays indicates that the IPO is well-structured and high-profile.
