The "suburban rut" of saturated markets plagues both broadband providers and healthcare systems (IDNs). My recent blog post explores how this shared challenge presents a golden opportunity. Leveraging telehealth and innovative solutions can make reaching rural populations profitable for both sectors. This shift not only benefits patients with increased access to care but also unlocks new revenue streams. Over the last 4-5 years I have been drawn into projects that involve and impact rural communities. For those of you who know me, I have spent decades developing broadband access markets working for global network equipment manufacturers, and more recently as part of Harbor Pilots. In my Connected Hospital experience working with Integrated Delivery Networks (IDNs) the notion of "access" had a slightly different meaning for healthcare providers but the challenge with access in broadband and healthcare revolves around population density and the cost to reach people. Working with Duke Tech Solutions we provided State and Local Governments with tools to optimize the $42 Bn Broadband Equity Access Deployment (BEAD) funding from the NTIA. Here's a blog post about the opportunity for Telecom providers and IDNs to look at rural communities as an attractive market where they can retreat from the intense competition in saturated suburban markets. Here is a disruptive idea we have been working on: Moving payments for ED clinicians from fee-for-service to global budgets. There is an opportunity to potentially test this in Maryland if we can get the stakeholders aligned. Walmart is exiting healthcare. This doesn’t surprise me. Walmart was never designed for healthcare, it was designed for retail. Everyone wants a piece of the $4T pie, but it’s a pie full of indigestible ingredients that would make the biggest companies choke. Especially companies that were never built for healthcare. - fee-for-service doesn’t work, especially with very low fees - relying on legacy payors doesn’t work - using insurance for primary care doesn’t work - not paying providers enough doesn’t work - having a clinic next to gardening supplies doesn’t work Throwing millions at healthcare doesn’t automatically work with a large base that comes to you for something else. I believe a properly built and well-executed healthcare startup that “thinks differently”, will eventually move the needle on improving the American healthcare experience. It’s coming, and will surprise everyone.

One key reason why healthcare technologies like AI, telemedicine, and software haven't structurally lowered costs or broadened access for the entire system after decades of experimentation (excluding pure financial engineering of payer contracts) is the limited reimagining of care delivery itself, with asynchronous telemedicine as a notable exception. In the same way that digitizing physical paper failed to exploit the capabilities of a new computing medium for word processing, hosting physician-patient conversations over video does not transform the actual delivery of care. In telemedicine, many of the inputs (e.g., clinician time) and therefore cost structures of in-person care persist, which is why we often see fee-for-service telemedicine companies struggle to achieve economies of scale (i.e., growing revenues non-linearly while growing costs linearly). Until we deconstruct the components of a medical appointment – from intake and triage, to chart review, history gathering, physical examination, diagnosis, treatment, and follow-up – we will continue to be underwhelmed by the disconnect between the transformative potential of these technologies and real-world patient/system impact. I’m optimistic that the emergence of reliable and performant medical AI agents will urge us to perform this deconstruction and redesign the fundamental building blocks of a “medical appointment.”

Digital health is reshaping traditional healthcare paradigms. From telemedicine platforms to wearable devices, start-ups are leveraging technology to enhance patient care, improve outcomes, and reduce costs. Investors are eyeing companies offering scalable solutions that streamline healthcare delivery, enhance accessibility, and empower individuals to take control of their health. Biotechnology is a hotbed of innovation, with breakthroughs in gene editing cell therapy and regenerative medicine promising to revolutionize treatment options for many diseases. Investors are allocating significant capital to buy tech startups with disruptive technologies and promising pipelines, recognizing the potential to address medical needs and generate substantial returns on investment. The era of one-size-fits-all medicine is giving to precision medicine, which tailors treatments to individual patients based on their genetic makeup, lifestyle, and environmental factors. With advances in genomics, molecular profiling, and biomarker identification, precision medicine promises to target therapies with greater efficacy and fewer side effects. Investors are betting on big startups pioneering precision medicine solutions, recognizing the value they bring to improving patient outcomes and reducing health costs. What are some of the best examples of how AI is helping tackle and improve patient care? How is AI impacting patients in real time? These were some of the questions we tackled in a wide-ranging panel discussion at the recent Fortune Brainstorm Health conference. I came away inspired by the knowledge that we are actively deploying the technology in a multitude of ways, and as it evolves, its impact will only continue to grow. In a discussion moderated by Fortune’s senior writer Erika Fry, several of us shared insights into how it’s being utilized for precision care in women’s health, to assist clinicians with patient note-taking, in medical devices that deliver therapeutics, in assessing mental health, and in Carrum’s case, how it enables rigorous provider quality evaluations and in identifying patients at risk for chronic health issues even before they’re aware of it. # My take is that we can harness AI for virtually every component of patient care, but the human element is still key for the critical context we need to make decisions, and this is where care concierges who pull all the pieces together and steer the patients matter the most. AI has the potential to dramatically enhance the care concierge team's performance and help them serve patients more effectively. # Second, the challenge with healthcare claims data is that it’s not a clean set like it is in other industries like financial services. So cleaning up claims data is a project in itself. With patients facing critical health issues, weaving data into the mix needs to be done with care so it truly enhances the patient experience. We use AI to predict which members in a given population are on a path to an acute care event in the near future. This is where I see it truly making a difference - until now, health care has been sick care. But with sophisticated data analysis, we can upend that so health care is preventative care. Plus, data we currently generate from handling thousands of patients going through surgery and cancer care also informs all the potential ways to help future patients avoid pitfalls and navigate their care better. #fortunehealth #patientcentricai #patientexperience

Digital Health: Countervailing Forces… Consider this: over the past five years nearly $78 billion has been invested in digital health companies and yet just this past year over 20 million Americans were taken off the Medicaid rolls. Certain policy changes can be very damaging, to say nothing of other human-induced factors like pollution, climate change, wars, inadequate housing and education, and hunger which have staggering implications on health. At times it can feel like bailing water out of a sinking ship. The overall venture capital activity in 1Q24 was a relatively muted $36.6 billion across 2,882 companies for an average round size of $12.7 million, as compared to $51.6 billion and 4,026 companies in 1Q23. Notwithstanding that the level of investment activity in the healthcare technology sector has receded from the $30 billion highwater mark in 2021 to a more manageable $10 – $12 billion annual pace, the opportunities to “transform the business of healthcare” remain obvious and acute. Arguably, the 2021 hangover still lingers in the healthcare technology sector as companies that raised too much capital at too high a price work to sort out challenging cap tables. Out of the 2021 mosh pit of too many companies, it appears that the sector is in an “Emerging Winners” phase that will drive companies with narrow and/or incomplete offerings to combine or shut down. Digital Front Door is a term I would love to see our industry stop using. It's long past the time for our industry to recognize that we need to BE digital, not just have a digital option - because that's what people expect from us. Limiting digital to a front door also perpetuates one of the biggest issues we have in healthcare today; requiring people to already know they have an issue and knowing where to go to find care. There is no single off the shelf solution to move us from a sick care system to a health care system, but there is an open standards architecture that we should ALL adopt so that we can innovate faster, modernize experiences and bring all kinds of companies together to create a more intuitive and proactive consumer experience.

There’s a category we believe is getting too little attention when it comes to AI in healthcare: AI Clinical Documentation Integrity (CDI) While AI ambient scribes and autonomous medical coding solutions have gotten most of the headlines, we’d argue AI CDI is equally impactful when it comes to quality care and accurate billing. For those not familiar, CDI ensures that clinical documentation accurately reflects the patient’s medical condition and the provided care. Historically, CDI was a manual slog, with dedicated CDI specialists, nurses, and physicians painstakingly reviewing patient charts to ensure documentation supported the care delivered. Today, AI is reshaping this landscape. While it’s still early days, we’re seeing three primary approaches to applying AI when it comes to CDI: 1️⃣ CDI + ambient scribing—an AI ambient scribe and AI CDI assistant operate simultaneously, generating comprehensive notes and surfacing codes for physicians to review at the point of care. (Example: Ambience Healthcare’s AutoCDI) 2️⃣ CDI via real-time diagnosis support—diagnoses are suggested at the point of care based on clinical data in the EHR, with auto-generated documentation and audit trails. (Example: Regard) 3️⃣ CDI via an AI-driven post-coding / pre-billing review—the entire patient record is ingested and analyzed against what is coded, surfacing suggestions for revenue and quality opportunities. (Example: SmarterDx) To date, AI in healthcare has focused primarily on administrative use cases. With the increasing adoption of AI CDI solutions, we’re now taking a step closer to clinical workflows. As clinicians get used to AI-suggested diagnoses in the CDI context, it will be interesting to see if this increases their comfort with AI playing a role in clinical care.
