CHAI and the Ministry of Health, Zambia, have published a groundbreaking study in BMC Health Services Research, evaluating the first large-scale evaluation of the introduction of the non-pneumatic anti-shock garment (NASG). The NASG is a lifesaving compression device that helps women manage heavy blood loss resulting from childbirth. In places where health facilities are remote, it can buy new mothers time before they can get the help they need. This study marks the first large-scale implementation of NASGs, covering 143 public health facilities. The introduction of the NASG, combined with training, clinical mentorship in shock management, and support for robust emergency referral systems, significantly improved maternal health outcomes. Despite past progress, global maternal mortality rates have stagnated and even reversed in some high-burden countries. Significant progress requires the introduction and sustainable scaling of innovations and under-utilized products such as the non-pneumatic anti-shock garment (NASG). The NASG is a lightweight, washable, reusable, and affordable lifesaving first aid device for women suffering from heavy blood loss following childbirth. In Zambia, the Ministry of Health, in partnership with CHAI, successfully deployed the NASG in rural and underdeveloped areas. The introduction of the NASG, combined with training, clinical mentorship in shock management, and support for robust emergency referral systems, significantly improved maternal health outcomes. This initiative is now being sustained beyond the initial partner support. Tikur Anbessa Specialized Hospital (TASH) in Ethiopia saves US$8,700 monthly, since the government reallocated two oxygen plants from the emergency response center used at the height of the #COVID19 to the hospital. At the onset of the #COVID19 pandemic, the Ethiopian government swiftly turned the Millennium Hall into a COVID-19 response center, expanding its bed capacity from 360 to 1,000. With Bill & Melinda Gates Foundation and CHAI support, it procured two additional oxygen plants, installed bedside oxygen support for up to 130 beds, and trained over 2,000 health workers to address the surge in patients. As the pandemic threat abated, the government collaborated with CHAI to identify high-impact hospitals to relocate the equipment used at the Millennium Hall. One of those hospitals was Tikur Anbessa Specialized Hospital (TASH), the oldest and largest referral hospital in the country with more than 700 functional beds, that serves patients from all regions in the country. The Ministry of Health (MOH) in collaboration with CHAI moved the first plant, pipeline, and manifold systems to TASH and the second plant to the UN makeshift Hospital/Bulbula COVID-19 Treatment Center for the continued management of COVID-19 cases. After the government revised its treatment protocol for the integration of COVID-19 into routine medical care services, the Ethiopian Ministry of Health moved the second PSA plant to TASH to ensure the sustainable availability of medical oxygen in this referral hospital. Today, the two oxygen plants at TASH have greatly alleviated the long-standing issues with accessing medical-grade oxygen and financial strain, resulting in monthly savings of over US$8,700. This is money that the hospital management would have otherwise spent on procuring oxygen from the private sector. The collaborative effort between MOH and CHAI proves that with thorough planning and commitment, we can turn a crisis into an opportunity for sustainable improvement in healthcare infrastructure. Rwanda's food supplement venture dramatically cuts stunting among children 18-23 months by 27%. The local joint venture between the government and private investors was created in response to the country's high child malnutrition rate. Since its launch in 2016, it has quickly become a regional leader, becoming the first producer of fortified blended foods in Africa to meet the World Food Programme quality requirements. Produced locally, incorporating local agricultural products, and aligning with regional and local eating habits, the products are designed for young children and pregnant/lactating women. By the end of 2021, over 85% of children in the poorest households had consumed the fortified blended food product. In addition, the percentage of anemic and iron-deficient children decreased from 35 to 9 percent and 34 to 24 percent, respectively. These findings underscore the success of a comprehensive approach and highlight the positive impact of the Rwandan Government’s commitment to holistic solutions promoting healthy child growth and development. The journey of fortified blended foods in Rwanda is a testament to transformative change and the potential for global solutions to local challenges.
