Housing Tenancy and Sustaining Services are among the various Community Supports available to CenCal Health members. Through our provider and community partners, members at risk of becoming unhoused receive support to help them retain their housing and ensure it is safe. These efforts are all part of CalAIM, California’s multiyear initiative to transform Medi-Cal to extend care beyond medical settings, addressing complex life factors that impact health and wellness. Collaborating with our healthcare partners to ensure our members receive high-quality health services is at the core of our mission. Through the Quality Care Incentive Program, CenCal Health has distributed over $4 million in incentive funds to high-performing primary care providers for the first quarter of 2024. Community Supports are essential services for Medi-Cal members, focusing on providing well-rounded care beyond a doctor’s office or hospital. At CenCal Health, we are committed to helping our members with their health-related social needs. These needs are often tied to issues like food insecurity, inadequate housing, and transportation barriers. Through Community Supports, we are working with our partners to positively impact health outcomes and overall wellness for the people we serve in San Luis Obispo and Santa Barbara counties. Starting today, we are expanding our Community Supports with our partners at Access Tlc Caregivers, Oxnard Family Circle ADHC, Independent Living Resource Center (ILRC) and Libertana.
