An emergency nurse educator describes the effects of a pediatric medication error. She had inadvertently weighed the patient in pounds, not kilograms, leading to a weight-based dosing error. The patient was fine, but the event left lasting implications for the educator. Medication errors are one of the most common causes of unintended harm to patients, and one-third of those errors occur during administration.
