The Healing Hearts Program provides a complete assessment of a family’s needs and provides assistance in locating resources to meet those needs.
How it works
Our team of clinicians will schedule home, office or virtual visits for individuals or the entire family.
Bereavement Support
Families who’ve lost a beloved child to a life threatening illness face a unique and all-encompassing sorrow. Jacob’s Heart is always here as families ride the waves of grief.
How it works
Our Bereaved Parent Support Group meets on a monthly basis to help support families who have lost a child to cancer.
