Our mission is to improve the quality of life for children with cancer and support their families in the challenges they face. 

 

Every family within Jacob’s Heart is embraced as part of our extended family, fostering peer mentorship and support networks that help navigate the complexities of pediatric cancer together.

Our approach to care begins with a thorough assessment of a family’s needs. We recognize that no two paths look exactly the same. That’s why we make it our mission to meet families wherever they’re at - practically, socially and emotionally.

Cancer center‐based follow‐up among pediatric and adolescent/young adult cancer survivors: the role of a community‐based organization and the social determinants of health

 

Adherence to survivorship care is suboptimal among pediatric and adolescent/young adult (AYA) cancer survivors. We evaluated predictors of cancer center-based follow-up among pediatric/AYA cancer survivors, with an emphasis on social determinants of health (SDOH).
