Wondering why an athlete keeps having pain during motion after an injury? 🤕 💪 Functional ultrasound analysis is important since many injuries can only be observed during a sporting gesture. It allows us to understand why during some sports movements pain is not generated while during other movement it is. Dr. Jorge Ojeda Barrientos present the case of diagnosing a rupture in the biceps femoris with dynamic ultrasound images using ProbeFix Dynamic. Link to request full case below the summary ⤵️ 👨⚕️ Dr. Ojeda is a doctor and professor specialized in sports medicine. In his clinic Medicina Muscular he works with elite and olympic athletes of different sports. He is a consulting doctor for soccer Club de Futbol Rayos del Necaxa and a respected member of the World Boxing Council (WBC). 🤕 Dr. Ojeda explains a case study about a 25-year-old athlete who performs several high-performance sports. The patient presented pain at the level of the biceps femoris tract, due to an eccentric contraction of the biceps femoris during a sudden stop. Physical examination showed no increase in volume or clear change of skin color. The knee remained at the complete range of mobility. The strength was 4/5 on the Daniels scale; the muscle can contract and provide resistance, but when maximum resistance is exerted, the muscle is unable to maintain the contraction. The pain did not allow the athlete to train or be in matches. After rehabilitation therapy he quickly returned to play. However, pain persisted! 🦵With ultrasound at rest it is challenging to diagnose a muscle tear, because it tear can be confused with edema or muscle strain. During dynamic evaluation with ProbeFix the muscle tear became evident; the tear is well delimited and the actual size of the tear showed (> 60% of the short head biceps femoris). The lesion did not generate contact with the sciatic nerve. The muscle was affected transversely, making recovery and rehabilitation difficult. 🏋 The combination of ultrasound and ProbeFix enabled Dr. Ojeda to identify the tear and then investigate the conditions and size. Based on those insights he treated the patient and adapted the recovery program. Over 30 years he worked with professional teams. Besides his work at universities, he is president of SEECO Sociedad Española de Ecografía) and directs global courses on MSK ultrasound MUSOC2024 and more. 𝐏𝐚𝐭𝐢𝐞𝐧𝐭 𝐜𝐚𝐬𝐞 🦵 The case involves 43-year-old athlete who tore his biceps femoris with continued residual and persistent pain in the back of his thigh and a strength deficit. Because the rupture, signs of fibrous scar are observed during a resting ultrasound exam. Generally, this is not symptomatic but rather a normal repair process. However, due to the persistence of the patient's symptoms, nerve compression is suspected. This is a complication due to excess of connective tissue in the proximity of the sciatic nerve, causing compression and preventing normal mobility of the sciatic nerve. 𝐃𝐢𝐚𝐠𝐧𝐨𝐬𝐢𝐬 🚶 With ultrasound at rest, it is challenging to confirm the diagnosis, because at rest limitation of the sciatic nerve mobility is hardly observable. Also using MRI, the nerve compression during to movement is not visible. ProbeFix was applied for dynamic and function ultrasound examination. First, the healthy thigh to observe the normal mobility of the nerve in the anteroposterior plane during walking. Then of the injured muscle, in which he observed limited movement of the sciatic nerve only displacing in one direction due to the proximity of the fibrous scar to the sciatic nerve. 𝐓𝐫𝐞𝐚𝐭𝐦𝐞𝐧𝐭 💉 The combination of GE HealthCare ultrasound and ProbeFix Dynamic enabled Dr. Jimenez Diaz to confirm the hypothesis and identify the source of pain and strength deficit.
