A Youth’s Journey with COMPASS Peacebuilders
Behavior Change Model
We track each youth’s progress in six areas using an evidence-based behavior change model. Each youth’s journey is different and there are many ups and downs. Below is a real example of one youth’s experience working with COMPASS Peacebuilders.
Meet Manny
Manny is 18 and lives in Hartford. He has a history of justice involvement, street involvement, and is known to carry a weapon. Diego feels he would be a good fit for COMPASS, but Manny has no interest in joining.
Building Trust
After months of outreach, Manny begins to trust Diego. He’s contemplating his future and asks Diego to support him at a court date.
Basic Needs
Diego and Peacebuilder Chuck deliver Thanksgiving dinner to Manny. They check to ensure his basic needs are being met.
Making Plans
Manny’s Peacebuilder calls to congratulate him for finishing High School and to talk about his plan to engage in more programming at COMPASS.
Challenges
In Feb. 2022, Manny tells his PB about his new job. A month later, he calls to say he lost that job and ask for help making a plan.
Resilience
Working with his Peacebuilder, Manny continues working on his employment goals and succeeds at getting his driver’s license.
Crisis
Manny is stabbed in an altercation. This is a major setback. COMPASS Crisis Manager Larry Johnson meets him bedside to help coordinate his care.
Home Visit
The Crisis Team and Manny’s Peacebuilder conduct a home visit to bring Manny some groceries, toiletries, and wound care supplies. They also connect him with home health support.
Mental Health
Manny meets with Haley Jones, a COMPASS licensed social worker, to help address his trauma and develop new stress management skills.
Progress
In the first half of 2023, Manny celebrated the birth of his twins and has continued working through his recent trauma. He also signed up for the Transitional Employment Program to build new skills and focus on his goals.

Navigator Case Management System
We gather our data using NAVI, COMPASS’s Navigator Case Management System, gives Peacebuilders access to real-time data wherever they are.
Navi pairs a custom app with Zoho CRM, allowing Peacebuilders to access details about every interaction they have with youth, report on youth development, and respond in real-time to changes in a young person’s life. Navi provides a dashboard for every young person in our program, showing progress toward weekly engagement goals.
