Request a patient application packet by completing the form, and we will send you a PDF packet via email. Once we receive your completed packet, we will start the verification process with your insurance company. We also will review your medical history and assign you a patient advocate who will reach out to you with further information about the program and processes.
SSM Health Weight Management Services patient application packets are available for the programs in Illinois and Wisconsin.
