Privileged to participate in the webinar “Patient-Centered Innovation: Key Steps for Clinicians, Purchasers & Payers” organised by NEJM Catalyst. I shared that as our population ages and the prevalence of chronic disease increases, we need to enable our patients to adapt and self manage amidst life’s challenges. At SingHealth, Medical Social Workers from our National Heart Centre Singapore implemented the Local Area Network and Empowerment (LACE) initiative for patients with heart failure with reductions in readmissions and improved quality of life. They asked their patients three questions: • What is a good life to you? • How can you use your strengths and skills to meet your goals? • How can your family, friend and community support you in meeting the goals? This requires a shift in our care mindsets to start with what is strong rather than what is wrong. Rather than viewing our patients as just service users with problems, we should engage and activate them as contributors to explore and create possibilities for themselves and others around them. Agency, the ability of an individual or group to act with purpose is critical for this transformation. By listening and learning more about our patients and their stories, we seek to move beyond health literary to empowerment to activation (from "I know" to "I can" to "I do" to even "I lead") as they work towards their life goals. My thanks to our moderator Thomas Lee, MD for the invitation, and to my fellow panellists Naseem Almulla FPCC - CPXP - CPHQ and Sven Gierlinger for their astute insights during the lively discussion. We also spoke about how health can be affected by many other factors and how a smooth administrative process can help give patients a peace of mind and allow them to focus on their recovery.
