Healthcare Economics 101: "PROFIT MARGIN and Short-Staffing" Less salaries = More profit -- this cartoon shows you how it works. Every time a boomer doctor retires or quits and their slot is not filled PRONTO,the remaining doctors work ethic kicks in and they burn themselves out to cover the patient care demands The institution bumps up their profit margin And the manager gets a pat on the back for running a "tight ship" Know this:  There is a massive finanacial DISINCENTIVE to hire and staff appropriately as long as you keep showing up and doing the work Managers everywhere prey on our ethics to pad profits - not ALL managers, but the majority for sure Forcing doctors to play chicken, pushed to the point of threatening to quit if you don't get reinforcements It is an obnoxious, evil management habit ...and so unnecessary This short staffing policy cripples the organization's long-term viability in search of short term profits (and executive bonuses) DOCTORS =>> GET OUT OF THERE Quit that Job - It's a fantastic job market with LOTS of virtual options. I was lucky enought to have an MD partner who, when the org decided that dyads of a doc and an admin were the best way to lead, said "I don't know a thing about doing this; I know clinical leadership but not the rest; you'll have to help me." His doing that was enough for me to want to help him. We did OK.

Couldn't agree more. When I was promoted to part of the management team at a large hospital system, we were given financial goals, the easiest way to hit them was to cut staff. Improving efficiency and flow was hard work. So the non-clinical leaders often set arbitrary staff levels to cut, resulting in the savings demanded. Amazingly, I didn't stay long, especially since part of their savings plan was to cut my salary in favor of more (and potentially higher) incentive based income.

Dike, there's also a tipping point, and you are actively promoting it: when the next tranche of docs decides to "take this job and shove it" the whole system will break. Yay! The sooner the better! It's TIME to build the next paradigm: there's not that much time left!

So true. This antiquated playbook will no longer work. To their credit, both millennials and Gen Z physicians will NOT continue to line up in formation, put their heads down and plow,and continue to play this mind game.
