As a Social Worker, you will provide essential social work services to individuals and groups, ensuring comprehensive psychosocial assessments, discharge planning, and implementation of appropriate interventions. Your role involves exercising professional judgment to anticipate and address patient and family needs, collaborating closely with healthcare teams and community resources to enhance patient care and continuity.
Key Responsibilities
Conduct psychosocial assessments, develop social work goals, and implement therapeutic interventions tailored to patient needs.
Document all activities in accordance with department protocols.
Advocate for patient involvement in care planning and coordinate interventions with healthcare team members.
Collaborate with community agencies to support patients with financial, housing, childcare, and other psychosocial challenges.
Provide crisis intervention services and manage hospital resources effectively for patients requiring complex care.
Educate patients and families on Advance Directives and assist with their execution as needed.
Communicate effectively with patients, families, healthcare team members, and community agencies to resolve issues impacting continuity of care.
Uphold professional ethics and standards of the social work profession, promoting a constructive approach to problem-solving and quality patient care.
Participate in staff orientation and education programs, contributing to the understanding of psychosocial aspects of healthcare delivery.
Contribute to unit-based educational initiatives and collaborate with staff to improve service delivery and patient care quality.
Demonstrate age-specific care knowledge and skills, adhering to departmental policies and procedures.
Required Qualifications
Master's Degree in Social Work.
Licensed Social Worker in the state of IL.
Minimum of 1 year of experience in social work within a complex healthcare setting.
Skills And Abilities
Proficiency in applying social work principles and methods within healthcare environments.
Strong decision-making, team-building, time management, customer service, and conflict resolution skills.
Excellent verbal and written communication skills.
Intermediate computer skills including word processing, presentations, and email.
Commitment to maintaining required hospital-based certifications.
Physical Requirements And Working Conditions
Work in a patient care environment with exposure to potential hazards such as biological, chemical, and mechanical risks.
Must be able to sit, stand, speak, read/write, and walk throughout the workday.
May require occasional travel, with exposure to road and weather hazards.
