Position Summary
We are seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our home health team. This role involves providing comprehensive mental health services to a diverse client base, focusing primarily on individuals covered under Medicaid. The LCSW will be instrumental in assessing patient needs, developing treatment plans, and providing therapy to help clients manage a variety of challenges associated with physical health conditions, mental health issues, and socioeconomic obstacles.
Responsibilities
Conduct initial assessments and periodic re-evaluations of clients to identify their needs and develop tailored intervention plans.
Provide direct therapeutic services such as individual, family, and group therapy sessions.
Collaborate with other healthcare professionals to integrate mental health services with overall health care management.
Advocate for clients to ensure they receive necessary services, which may include coordinating with other agencies or providers involved in their care.
Document and maintain accurate and detailed client records in compliance with regulatory standards and confidentiality laws.
Assist clients and their families in understanding and navigating the complexities of the Medicaid system.
Engage in continuous professional development to ensure best practices in clinical social work and compliance with all state regulations and requirements.
Qualifications
Master’s degree in Social Work (MSW) and a current LCSW license valid in the state of Maryland.
Proven experience in mental health settings, preferably in home health or related fields.
Deep understanding of the Medicaid system and the specific needs of clients utilizing this insurance.
Strong interpersonal and communication skills, with the ability to work effectively with diverse populations.
Commitment to providing respectful and empathetic care to all clients.
Ability to work independently and as part of a multidisciplinary team.
Proficiency in electronic health records and case management software.
