Job Details

Are you a compassionate, experienced Social Worker with a background in hospice or palliative settings? We are seeking a Permanent Social Worker to join our dynamic healthcare team. This position involves offering psychosocial support to patients and their families, particularly those dealing with terminal illnesses and end-of-life care. The ideal candidate has a strong background in psychiatry and is adept at conducting comprehensive assessments. This role requires a minimum of 5 years of experience in a similar capacity, with a focus on empathetic, patient-centered care.

Responsibilities

As a Permanent Social Worker, you will be responsible for

• Providing emotional and psychosocial support to patients and their families, particularly those dealing with terminal illness and end-of-life care situations.
• Conducting comprehensive psychosocial assessments and developing individualized care plans based on these assessments.
• Collaborating with a multidisciplinary team of healthcare professionals to ensure the holistic care of patients.
• Navigating complex medical information and explaining it in a way that patients and their families can understand.
• Advocating for patients' rights and needs, ensuring they are being met throughout their medical journey.
• Providing crisis intervention as needed and facilitating access to appropriate mental health resources.
• Assisting patients and their families in understanding and making decisions regarding treatment options, advance directives, and other end-of-life decisions.
• Coordinating with community resources to ensure continuity of care post-discharge.
  
  

Qualifications

The ideal candidate for the Permanent Social Worker position will possess the following qualifications

• A Master’s degree in Social Work from an accredited institution.
• A minimum of 5 years of experience as a social worker, preferably in a medical or psychiatric setting.
• Experience working with patients dealing with terminal illnesses and end-of-life care situations.
• A deep understanding of psychiatric principles, including the ability to conduct comprehensive psychosocial assessments.
• Exceptional communication skills, with the ability to explain complex medical information in a way that patients and their families can understand.
• Strong advocacy skills, with a proven track record of advocating for patients' rights and needs.
• The ability to work effectively as part of a multidisciplinary healthcare team.
• A compassionate, patient-centered approach to care, with a genuine desire to support patients and their families during challenging times.
