This position offers full remote work after completing 2-3 weeks of training in Manhattan, NY. Qualified candidates must hold a NYS-RN License and have previous experience in MLTC RN Care Management. Candidates must reside in downstate NY or NJ, and be available to commute to Manhattan, NY for initial training and occasional in-office meetings.
Role Overview
The RN Care Manager provides clinical support and oversight to ensure members receive safe and effective care in the community.
Qualifications
RN (BSN preferred). Must hold a current, valid, and unencumbered license as required.
Experience as a NY licensed RN in home care, acute, sub-acute, LTC, or MLTCP settings.
Maintain state and federal compliance requirements for licensure and employment.
Bilingual, UAS certified, and care management knowledge preferred.
Reside in Downstate NY, Westchester, Long Island, or Northern New Jersey.
Essential Responsibilities
Proficient in article 49 regulation.
Authorize services, create care plans, and coordinate care for high-risk populations.
Coordinate hospitalizations, skilled services, and care transitions.
Manage Medicare, Medicaid, And Other Insurance Benefits Coordination.
Conduct monthly call assessments and follow-up on care changes.
Coordinate post-discharge services as required.
Additional duties as assigned. Bilingual proficiency (Spanish/Cantonese/Mandarin) is a plus.
