Are you an experienced Registered Nurse looking for a rewarding remote opportunity? We are seeking dedicated RN Care Managers to join our team, providing exceptional care to Managed Care Plan members. This position is fully remote after an initial 2-3 weeks of training in Manhattan, NY.
Key Responsibilities
Develop and implement personalized care plans for high-risk populations.
Authorize services and coordinate care across the continuum, ensuring members receive safe and effective community care.
Assess and manage hospitalizations, skilled services, and transitions between home and healthcare facilities.
Handle coordination of benefits for Medicare, Medicaid, and other insurances.
Conduct monthly assessments and follow up on changes in medications, diagnoses, or significant events.
Coordinate post-discharge services and perform other duties as assigned.
Qualifications
Registered Nurse (BSN preferred) with a current, valid, and unencumbered NYS-RN license.
Experience in home care, acute, sub-acute, or long-term care settings, or managed long-term care (MLTC).
Bilingual skills and UAS certification are highly desirable.
Must reside in downstate NY, Westchester, Long Island, or Northern New Jersey, with the ability to commute to Manhattan for initial training and occasional meetings.
Proficiency in Article 49 regulations is required.
Commitment to maintaining state and federal compliance requirements.
