We are seeking a compassionate and dependable registered nurse to join our home health care team in the Fort Collins, CO area as an RN Case Manager. You will play a crucial role in developing care plans and delivering skilled nursing care to families with children who have intellectual and developmental disabilities (IDD).
In This Role You Will
Visit patients' homes every 60 days to assess and update care plans.
Provide in-home care to children and monitor their progress.
Educate and support parents on their children's health needs.
Utilize telehealth to enhance care coordination and communication with the care team and families.
Advocate for children's health outcomes and contribute to our mission.
Requirements
Registered Nurse with at least 2 years of experience and current Colorado state licensure (or compact license).
Experience in home care, with preference for those who have trained family CNAs.
Comfortable in a fast-paced, startup environment.
Reside in or willing to relocate to Fort Collins, CO.
CPR certification required.
Pediatric And Home Care Experience Essential.
BLS certification required; PALS certification preferred.

Monday
Daily Schedule:
Mandatory nurse meeting in Denver office from 9am to 10:30am.
Admin day for nurses to work from home and complete tasks.
Tuesday-Friday
In-person recertifications (up to 3 per day).
In-person start of care assessments (up to 3 per week).
Remote work for charting, family communication, and case management.
