Position Overview

As an RN Case Manager, you will partner with clinical teams to provide complex case management and strengthen the connection between patients and primary care physicians/medical practice staff. You will be responsible for assisting patients in achieving their highest level of wellness by managing a specific patient population with complex medical conditions, socio-economic challenges, and/or mental health co-morbidities.

Key Responsibilities

  • Comprehensively assess healthcare and psychosocial needs of patients
  • Review and amend patient navigator interviews
  • Develop, implement, and maintain individualized care plans
  • Provide evidence-based clinical interventions
  • Collaborate with patient navigators to manage non-clinical elements of care plans
  • Conduct ongoing patient outreach and monitoring
  • Develop patient-specific escalation plans with primary care physicians
  • Provide education on conditions and treatment options
  • Advocate for patients and assist with accessing clinical services/resources
  • Maintain up-to-date documentation in electronic medical health records
  • Utilize evidence-based clinical protocols and quality metrics

Qualifications

  • Licensed Registered Nurse in good standing
  • 3+ years of RN Case Management experience in an outpatient setting
  • CCM certification preferred (must obtain within one year of employment)
  • Supervisory experience preferred
  • Excellent written and oral communication skills
  • Strong computer skills and ability to learn multiple EMR systems
  • Knowledge of accreditation and quality standards (e.g., NCQA, HEDIS)

Additional Skills

  • Bilingual in English and Spanish (preferred)

Location and Travel:

This is a remote position based in the Philadelphia or Harrisburg, PA area. Some travel (10-15%) to clinical sites in these regions may be required.
