Position Overview
As an RN Case Manager, you will partner with clinical teams to provide complex case management and strengthen the connection between patients and primary care physicians/medical practice staff. You will be responsible for assisting patients in achieving their highest level of wellness by managing a specific patient population with complex medical conditions, socio-economic challenges, and/or mental health co-morbidities.
Key Responsibilities
Comprehensively assess healthcare and psychosocial needs of patients
Review and amend patient navigator interviews
Develop, implement, and maintain individualized care plans
Provide evidence-based clinical interventions
Collaborate with patient navigators to manage non-clinical elements of care plans
Conduct ongoing patient outreach and monitoring
Develop patient-specific escalation plans with primary care physicians
Provide education on conditions and treatment options
Advocate for patients and assist with accessing clinical services/resources
Maintain up-to-date documentation in electronic medical health records
Utilize evidence-based clinical protocols and quality metrics
Qualifications
Licensed Registered Nurse in good standing
3+ years of RN Case Management experience in an outpatient setting
CCM certification preferred (must obtain within one year of employment)
Supervisory experience preferred
Excellent written and oral communication skills
Strong computer skills and ability to learn multiple EMR systems
Knowledge of accreditation and quality standards (e.g., NCQA, HEDIS)
Additional Skills
Bilingual in English and Spanish (preferred)
Location and Travel:
This is a remote position based in the Philadelphia or Harrisburg, PA area. Some travel (10-15%) to clinical sites in these regions may be required.

Base salary range: $80,000 to $85,000 per year
