Position Overview

As an RN Case Manager, you will collaborate closely with clinical teams to provide complex case management and enhance the connection between patients and primary care physicians or medical practice staff. You will be responsible for managing a specific patient population, including those with complex medical conditions, socio-economic challenges, and/or mental health co-morbidities. Your role will involve optimizing patient health through comprehensive assessment, personalized care planning, coordination of services, and continuous monitoring and evaluation.

Location: Remote position with occasional travel (10%-15%) to clinical sites in Philadelphia and Harrisburg, PA. Local applicants preferred.

Responsibilities

Conduct comprehensive assessments of healthcare and psychosocial needs for patients.

Develop, implement, and maintain individualized care plans in accordance with internal protocols and accreditation standards.

Provide evidence-based clinical interventions based on care plan goals.

Collaborate with patient navigators to manage non-clinical elements of care plans (e.g., appointments, transportation, food services).

Conduct ongoing outreach to patients, monitor patient data, and adjust care plans as needed.

Develop patient-specific escalation plans with primary care physicians for acute, non-emergent exacerbations of chronic conditions.

Educate patients on their conditions and treatment options.

Serve as an advocate for patients, assisting in accessing clinical services and resources.

Follow up on service or resource referrals and maintain updated documentation in electronic medical health records.

Utilize evidence-based clinical protocols and guidelines to achieve defined quality metrics.

Requirements

Licensed Registered Nurse (RN) in good standing.

3+ years of RN Case Management experience in an outpatient setting.

CCM certification preferred; must obtain CCM within recommended timeframe.

Supervisory Experience Preferred.

Excellent written and oral communication skills to interact empathetically with patients, families, and interdisciplinary teams.

Proficiency in computer skills, including multiple EMR and Case Management documentation systems.

Ability to meet accreditation and quality standards (e.g., NCQA and HEDIS).

Preferred Qualifications

Bilingual in English and Spanish.
