Job Title: Program Specialist II-Social Worker

Duration: 3 Months (Possible Extension)

Location: FL, Remote (Field worker in Lee County Area)

Shift: Standard Shift

Job Summary

  • Conduct screening and risk assessment interventions per program guidelines.
  • Assist with non-medical aspects of the member’s care, including referrals to community resources.
  • Note:
  • Looking for local candidates of Lee County Area, FL (Labelle, Lehigh Acres, Alva, Fort Meyers, or Naples).
  • Candidate must be licensed master social worker OR licensed social worker OR licensed practical nurse.
  • Candidate required to do Field worker making face to face visits at members home or other agreed upon public locale.
  • Commuting will be within 1-2 hours of the candidate location and travel expenses will be covered by the company.

Job Responsibilities

  • Coordinating care for special needs families on the CMS plan.
  • Works on non-acute members.
  • Caseload compliance per CMS contract. field visits, assessments, care plans.
  • Identify special needs members through the completion of health screens and other resources Work with community outreach/member advocates to coordinate member care.
  • Educate providers and community resources on program components and available support services.
  • Educate members with special needs to foster compliance with program and positively impact outcomes Conduct site visits as appropriate for programs and provide support to other special programs.
  • Develop and modify care plans in conjunction with members, member’s family and managing physician.
  • Development of plan specific literature and education materials in conjunction with medical director and corporate oversight.

Education/Experience

  • Bachelor’s degree in Social Work, Nursing, Health, Behavioral Science or Equivalent Experience.
  • Experience in a managed care environment preferred. 2-4 years of experience
