We are seeking a full-time Patient Navigator whose expertise and training can help support and guide members to make informed decisions on their healthcare journey. The Patient Navigator is part of the Navigation Services team and plays a crucial role in facilitating and coordinating personalized navigation services for Genomic Life Members. This position involves providing initial triage for Genomic Life services, ensuring accurate documentation of patient data, and connecting patients with appropriate resources to meet their practical, social, physical, emotional, and spiritual needs. The Patient Navigator works in collaboration with the Clinical Services Supervisor and Clinical Navigators, demonstrating active listening skills, empathy, and a strong understanding of evidence-based genomic information.

Job Type: Full-time

Salary Range: $60-80k base

Responsibilities:

  • Provide initial triage for Genomic Life services, identifying patients' genomic navigation needs.
  • Act as the primary point of contact for members seeking assistance with accessing benefits, promptly addressing inquiries, and resolving any issues they encounter.
  • Handle member concerns and complaints with professionalism and empathy, actively listening to their feedback and working towards mutually beneficial resolutions.
  • Demonstrated ability to effectively prioritize tasks and maintain impeccable organization in a fast-paced environment, ensuring deadlines are met and objectives are achieved efficiently.
  • Initiate personalized genomic navigation services based on patient requirements.
  • Maintain a working knowledge of evidence-based genomic information to ensure quality-of-care coordination and engaging in ongoing professional development.
  • Ensure accurate triage documentation and verification of all required patient data using pre-designated tools.
  • Participate in tracking and monitoring metrics and outcomes, collaborating with administration, and reporting findings to relevant stakeholders.
  • Communicate patient information appropriately to the Genomic Life Clinical Services team and within the healthcare team.
  • Identify potential and realized barriers to care and facilitate appropriate and credible resources responsive to members' needs, considering factors such as reading level, health literacy, culture, language, and desired amount of information.
  • Promote, and advocate prevention behaviors, early detection, screening, and healthy behavior education.
  • Schedule member appointments for Clinical Navigator and Genetic Counselling appointments as appropriate.
  • Track medical specimens and request medical records to facilitate seamless care.
  • Utilize active listening skills and remain solution-oriented when interacting with patients, families, and members of the healthcare team.
  • Demonstrate empathy, integrity, honesty, and compassion in difficult conversations.
  • Demonstrate cultural humility, sensitivity, and responsiveness to diverse patient populations, including gender/gender identity, age, culture, race, ethnicity, religion, abilities, and sexual orientation.
  • Participate in collaborative team communication escalating operational challenges to optimize service delivery.
  • Adhere to all HIPAA guidelines for privacy and confidentiality practices.

Qualifications:

  • Bachelor's degree in a relevant field (biology, chemistry, or related field).
  • Knowledge of evidence-based healthcare, and patient navigation principles.
  • 5+ years Oncology Patient Navigation experience.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively in a multidisciplinary healthcare team.
  • Sensitivity to cultural diversity and the ability to navigate diverse patient populations.
  • Familiarity with HIPAA guidelines and practices.
  • Must have experience in the medical industry.
  • Must be a self-starter, critical thinker, detail-oriented, and highly organized.
  • Proficient in Microsoft Office and Zoom and ability to learn new software (Salesforce.com).

Required Education:

  • Bachelor's degree in a relevant field (biology, chemistry, or related field), OR AS degree with 6+ years of healthcare related experience.

Preferred Qualifications:

  • 1+ year of telephone and/or triage experience.
  • Navigation Training -either of the following
    • GW Patient Navigation Training
    • ACS LION Patient Navigation Training
  • Leadership experience.
  • Excellent verbal and written communication skills.
  • Good listening and interpersonal skills.
  • Strong attention to detail.
  • Salesforce experience a plus.
  • Fluency in Spanish or other languages is a plus.
