Patient Care Navigator - Remote | WFH

About The Role

Join us in putting healthcare on a better path! This fully remote position involves supporting patient care while collaborating closely with medical practice staff and our dedicated Case Management team. You will strengthen connections between patients and their primary care physicians, enhance patient health and well-being, and play a pivotal role in improving communication within medical practices.

Key Responsibilities

Manage a targeted population of patients, working remotely with medical practice staff.

Independently function as an extension of medical practices, collaborating closely with our team.

Assist patients in achieving better health outcomes by addressing their questions and concerns, connecting them with medical practices, and implementing care plans.

Document patients' medical history and issues accurately in electronic health records (EHR).

Build trusting relationships with assigned medical practices and within our organization.

Meet Key Performance Indicators to ensure positive patient health outcomes and reduce medical costs.

Utilize effective communication channels (calls, texts, portals, emails) to engage with high-risk patients.

Escalate medical issues to the appropriate healthcare professionals when needed.

Occasional travel to supported primary care physician offices may be required.

Requirements

Passion for patient coordination and serving individuals with complex medical and social needs.

Empathy and proficiency in motivational interviewing and critical listening skills.

Strong conversation and documentation skills, with experience in medical terminology and primary care functions.

Knowledge of social determinants of health; bilingual (English and Spanish) skills preferred.

Certification as a Medical Assistant or Community Health Assistant with relevant experience.

Familiarity with electronic health records (EHRs) and care management databases.

Ability to balance competing priorities while maintaining attention to detail.

Comfortable working independently and as part of a remote team.

Familiarity with EPIC, Athena, Cerner, or similar EHR systems.

Excellent written, visual, and verbal communication skills.
