Position Overview
As a Hybrid Nurse Case Manager, you'll play a pivotal role in assessing and coordinating care for our members, with a mix of remote and in-person interactions. Your responsibilities will include:
Conducting comprehensive assessments to determine the need for case management intervention.
Collaborating with healthcare providers and stakeholders to develop and implement personalized care plans.
Educating recipients, families, and caregivers on available community resources and services.
Evaluating the effectiveness and efficiency of care plans and making necessary revisions.
Utilizing your clinical expertise to optimize the quality of care delivered.
Key Highlights
Schedule: Monday to Friday, 8:00 am to 4:30 pm (no nights or weekends), with travel time included.
Benefits: Full-time employees are eligible for comprehensive benefits, including health coverage, paid time off, and a 401(k) with company match after 90 days of employment.
Compensation: Competitive compensation starting at $39.92 per hour.
Requirements
Active VA Registered Nurse license.
Minimum of 3 years of clinical experience (medical surgical, behavioral health, etc.).
Preferred experience in case management; home health/home care experience is a plus.
Proficiency in computer skills to navigate relevant software and systems.
