We are seeking a forward-thinking, organized, and energetic Nurse Care Manager to join our team. As a Nurse Care Manager at our medical group, you will play a crucial role in developing, implementing, and delivering impactful change. Working closely with our population health teams, networked facility partners, primary care providers, and practice operations, you will provide extensive care coordination for our highest-risk patients.
Responsibilities
Develop trust and build rapport with a select patient population, aiming to reduce high-cost utilization and improve clinical outcomes.
Collaborate across various teams including Payer Contracting, Care Management, Performance Management, and Analytics.
Foster strategic relationships with payers to transition from fee-for-service to fee-for-value based care.
Engage patients via telephonic, in-office, facility visits, or as needed.
Serve as a liaison between the PCP, patient, and payer contract.
Attend and contribute to operational meetings with payers.
Assist patients in accessing appropriate care sources.
Conduct holistic assessments (medical, social, economic, behavioral) to identify opportunities and barriers for improved outcomes.
Collaborate with patients and care center teams to develop personalized care plans.
Conduct in-person and telephonic follow-ups with high-risk patient populations for education and goal setting.
Ensure all tasks adhere to established protocols and standing orders, with RN or LPN supervision where applicable.
Utilize Athena and Applications to identify high-value opportunities, decrease utilization, and close quality gaps.
Maintain a courteous and helpful manner when interacting with patients, physicians, and colleagues.
Participate in special projects and other assigned duties.
Qualifications
Bachelor's degree in Nursing (or equivalent) preferred.
Minimum 3+ years of case management or care coordination experience.
Active unrestricted nursing license in VA, DC, MD, or ability to obtain (if needed).
Strong computer skills and proficiency in EHR systems.
Compliance with HIPAA rules and regulations.
Compensation
Salary range: $52,000.00 to $65,000.00 base pay annually, exclusive of bonuses or benefits.
Eligible for an annual bonus targeted at 10% based on performance.
