The LPN Clinical Data Reviewer will play a crucial role in delivering high quality patient monitoring reports for the Care Management service line. They are responsible for reviewing patient medical data from electronic medical records (EMR) system to compile Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) monthly patient reports.
PRIMARY RESPONSIBILITIES:
Adheres to Chronic Care Management regulatory compliance as defined by CMS, ensuring the integrity of data and documentation meets the internal standards set by Care Management services as defined by Tapestry Care Management department.
Conduct thorough assessments of patients' health conditions based on the collected data, identifying any potential changes or abnormalities, escalating as appropriate.
Responsible for the accuracy of all data points based on access to facility records.
Compiles a complete initial report that will be reviewed by the NP/Provider Clinical reviewer to develop a comprehensive care plan.
Utilize our proprietary remote monitoring platform to review and analyze patients' physiological data, including vital signs, activity levels, and other relevant metrics as defined in the playbook
Successfully meets organizational and departmental excellence standards
Cultivates a culture of accountability with internal and external stakeholders, to facilitate open communication program enhancement.
Performs all duties and responsibilities and adheres to the Nurse Practice Act.
Works with Tapestry IT and Data Scientists in resolving mismatch patient information, and discrepancies
Ensures timelines are met
Protects patient information and adheres to standard of confidentiality with all patient information
Contributes and participates in the development of performance and continuous quality improvement activities within the scope of Care Management.
Participates in routine office hours, provides TEAMS CCM channel oversight, and organizes meetings for small groups of Data Quality Reviewers
Performs monthly Data Quality Audits, utilizing the departmental scorecard, delivers feedback and develops improvement plans

MINIMUM REQUIREMENTS:
Current and valid LPN license
Minimum of 3 years’ experience as an LPN
Strong clinical background with experience in remote patient monitoring, telehealth, or similar settings
Proficiency in using remote monitoring platforms and electronic health records systems
Proficient with Adobe, Microsoft Word, Excel, and video conferencing platforms and Dashboard type platforms.
Familiarity with HIPAA (Health Insurance Portability and Accountability) regulations and patient privacy standards

Experience in working with team members to promote ideas, issues, and initiatives within a constructive group framework
Must always represent the organization in a positive and professional manner

Ability to educate internal and external customers on care management services, and Tapestry care Management service line providers as part of their assigned facilities
