Job Summary
We're seeking a Licensed Clinical Social Worker (LCSW) who will play a pivotal role in providing clinical consultations, evaluating, and serving as a valuable resource to Lead Care Managers and management.
Key Responsibilities
Provide support to Lead Care Managers serving adult and pediatric behavioral health members.
Conduct timely clinical consultations for behavioral health members' care management.
Evaluate, adjust, and execute member Care Management Plans as required.
Effectively communicate recommendations, learning needs, and healthcare teachings to Lead Care Managers and members, ensuring thorough documentation in the electronic health record (EHR).
Offer education to members and their families on health-related topics, treatment options, and self-care strategies, both in-person and via telephonic channels.
Assist in Discharge Planning, particularly in coordinating the discharge process for behavioral health care members leaving inpatient psychiatric facilities.
Facilitate access to behavioral health providers when necessary.
Support in New Hire Orientation training.
Coordinate, implement, and monitor options and services to fulfill at-risk members' healthcare needs through seamless communication and coordination with providers.
Provide individual and group supervision to Associate Clinical Social Workers (ACSWs) employed with our organization.
Adhere to all departmental and company-wide policies and procedures.
Attend weekly Multi-Disciplinary Case Conferences and SCR, contributing clinical recommendations.
Maintain compliance with safety and injury prevention policies and regulations.
Qualifications
Perform other duties as assigned or required.
Education: Master's in Social Work
Experience: Minimum 3 years' experience as an LCSW in an acute care setting.
Proficiency with an EHR/EPM; experience with eClinical Works (ECW) is preferred.
Previous experience in an internal medicine clinic is desirable.
Language: Fluent in English (both written and verbal); bilingual proficiency is a plus.
License/certification: Possession of a valid, unrestricted California state license.
Valid BLS certification from the American Heart Association.
Skills Required
Proficient with computers, email, virtual platforms, Excel, and other Microsoft Office programs.
Excellent verbal and written communication skills, with the ability to convey information clearly and effectively.
Strong problem-solving abilities, utilizing logic and information to develop and implement solutions.
Ability to work independently and execute assignments within established policies and procedures.
Physical Requirements
Associated with office work and driving, including operating a computer and other office equipment, occasionally lifting office equipment weighing up to 15 pounds, and traveling to meetings or different office locations as needed.
