A Brief Overview

As a Hospice Liaison, your role is to coordinate and facilitate referrals to Supportive Care, including Hospice and Palliative Care, based on admission criteria, recommend alternate plans of care as appropriate, and improve access to Hospice and Palliative services for individuals and families with a terminal or life limiting illness. Serves as on-site Hospice resource to assist facility staff that refers patients to Hospice Services. Works with hospitals, nursing homes, SNFs, ALFs, and community-based leaders and organizations to further educate and coordinate hospice and palliative services with patient and families in need. The Hospice Liaison will be responsible for developing and maintaining relationships with internal and external referral sources, evaluate the patient to ensure appropriateness of transfer, educate patients and families on the hospice care and services, and act as a liaison for the Hospice Team to any referring provider and/or facilities. Travel as necessary to evaluate referrals and establish professional relationships with potential referral sources

What you will do

  • Obtains and documents referral information, including current medications and treatments, physician, patient and primary care person’s agreement to Hospice Services
  • Works with Hospice Medical Director and Interdisciplinary Team to evaluate patient referrals to determine appropriateness for admissions to the Hospice Program and/or other Supportive Care services. Evaluates referrals based on admission criteria and recommends alternative plans of care as appropriate
  • Provides consultation and counseling to referrers, patients and/or families about hospice care and end of life care options. Coordinates with the Hospice Team to ensure that needed equipment and services are in place prior to patients' admission for hospice care. Identifies appropriate providers for durable medical equipment and supplies, obtaining authorization as needed
  • Notifies Hospice Team of patient's acceptance for services and obtains signed hospice consents, provides intake data, essential medical background, preliminary care plan and medical orders, necessary to facilitate admission to the hospice program
  • Supports case managers in timely referrals of patients to hospice services
  • Collaborates with Intake Team on insurance coverage for requested hospice services and informs patients and families of sources and extent of coverage
  • Establishes and promotes ongoing collaborative relationships with referral sources, institutional personnel, and other facilities to ensure the coordination of appropriate referrals
  • Acts as a liaison to patients and families during the transitional period between patient's discharge and the initiation of Hospice Program services
  • Participates in Interdisciplinary Team meetings for the purpose of information exchange on patient care planning
  • Participates in the Quality Improvement initiatives related to improved quality patient care
  • Participates in special projects and performs other duties, as requested by Administration
  • Maintain familiarity with the Medicare Hospice Benefit and educate patients, families and healthcare professionals
  • Generates appropriate referrals to Hospice, Palliative and Villa Marie Claire from current and new referral sources
  • Establishes and maintains professional and productive relationships with all referral sources, including physicians, nursing homes, assisted living facilities, hospital personnel, including discharge planner and/or case managers and other appropriate referral sources
  • Maintains standards of high quality customer service, and shows respect to all constituents, both internal and external
  • Participates in evening/weekend calls for admissions as required, in a competent and responsive manner

Education Qualifications

  • Bachelor's Degree BSN Preferred or
  • Master's Degree MSW

Experience Qualifications

  • Two years in a hospital or related health care setting Required
  • 1-3 years Direct patient care Preferred

Knowledge, Skills, And Abilities

  • Knowledge of hospice criteria and services
  • Able to work well independently and communicate with the healthcare team
  • Excellent communication, negotiation, and public relation’s skills
  • Ability to market effectively referral sources, and the community
  • Counseling skills, providing education and support to terminal patients and families. Flexible and cooperative in fulfilling all obligations

Licenses and Certifications

  • DL NUMBER - Driver License, Valid and in State Valid driver’s license and means of transportation Upon Hire Required and
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure State of New Jersey Upon Hire Preferred
