Health care providers perform primary care and acute visits in homes, ALF's, and ILF's. This is a salary-based position, with the opportunity to receive a monthly bonus. This is a full-time 7on/7off 12-hr position (7:00AM-7:00PM) (Monday-Sunday).

Requirements

• Active APRN license
• Excellent communication and interpersonal skills with the ability to build strong relationships at all levels of the organization.
• Strong problem-solving abilities
• Ability to work effectively under pressure.
• Ability to travel and perform care in a home setting
