Director, Patient and Care Team Solutions

As the Director of Patient Care Solutions, you will lead a dynamic team of product managers, product owners, and scrum teams. Your mission will be to ideate, design, build, and oversee a portfolio of digitally enabled services that seamlessly connect patients with clinicians, engagement specialists, primary care physicians, and nephrologists. This role is critical in delivering an integrated care experience that fosters better health outcomes and higher patient satisfaction. 

Key Responsibilities: 

  • Lead and inspire a team of product managers, owners, analysts, and scrum teams, to innovate digitally enabled services. 
  • Spearhead the development and implementation of solutions that improve health outcomes for patients and everyone that supports them. 
  • Oversee the lifecycle from ideation through deployment and ongoing optimization. 
  • Ensure that digital services align with clinical guidelines, legal, privacy, compliance and regulatory requirements while delivering a cohesive patient experience. 
  • Collaborate with cross-functional teams, including Technology, Design, Research, Healthcare Operations, the Chief Medical Office, and external partners, to ensure that digital solutions meet operational, clinical, and customer satisfaction goals. 
  • Work with Marketing, Growth and Strategy to identify the biggest growth opportunities. 
  • Drive continuous improvement by incorporating feedback into future enhancements. 
  • Stay abreast of industry trends and technology advancements that can accelerate growth. 

Qualifications: 



  • 10+ years of experience leading successful product teams in a rapidly growing healthcare or healthtech business of significant size. 
  • Demonstrated success in startups from 0-1 and scaleups from 1 to many. 
  • Extensive experience in managing the full product lifecycle from ideation through sunset. 
  • Strong understanding of clinical processes and the healthcare ecosystem, particularly in relation to chronic disease management. 
  • Excellent communication and collaboration skills, capable of working effectively with diverse stakeholders. 
  • A strategic thinker with a track record of translating strategy into actionable, high-impact solutions. 
  • Advanced degree in Health Administration, Business, or a related field is preferred.
