All Metro Health Care, a Modivcare Personal Care Service, is looking for a Per Diem RN Clinical Supervisor. You will have the opportunity to make a meaningful impact on the lives of our clients and foster the professional growth of our home health aides.

• Pay per Visit
• Initial Admission: $80
• Resumption of Care: $50
• Re-certification: $65
• Supervisory Vist: $35
• Post fall Visit: $50
• Schedule: Days, Evenings, & Weekends
• Visiting Client Territory: Albany area
• Office: 350 Northern Blvd., Suite 206, Albany, NY 12204

You will...

• Performs on-site supervisory and teaching visits to evaluate caregivers
• Provides clinical orientation to caregivers on assigned home care cases.
• Participates in the training of new nurses and in-services for field staff.
• Performs initial nursing assessments
• Counsels patient/caregiver in meeting nursing and related needs.
• Assigns and coordinates services provided.
• Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient’s condition/needs.
• Prepares clinical progress notes ensuring any changes in the patient’s condition is reported
• Monitors clinical records for complete and accurate documentation.

We are excited to speak to someone with the following…

• Registered Nurse (RN)
• CPR Certified
• 1 year of Home Care experience required
• 1 year of RN experience in a skilled care setting
• Valid NYS Driver’s License, insurance, registration, and reliable car
