As a Clinical Manager, you will provide remote or on-site coverage at care centers under the direction of the AVP Clinical. You will coordinate closely with Directors of Operations to ensure high-quality patient care, fiscal responsibility, and compliance with policies and regulatory requirements. This role is pivotal in measuring, monitoring, and managing patient care quality and organizational clinical performance.
Responsibilities
Coordinate scheduling to ensure patient needs are met, collaborating with the Patient Services Coordinator and staff.
Manage payor verification and pre-certification requirements, maintaining communication with payors and responding to inquiries.
Review clinician documentation, including Oasis data collection, to ensure accuracy and compliance with care delivery standards.
Provide constructive feedback to clinicians on documentation, skills, and behavior.
Assist clinicians in setting patient goals and developing comprehensive Plans of Care (POC).
Monitor patient progress and adjust care plans in collaboration with clinical staff.
Facilitate patient care conferences and ensure effective communication among team members.
Participate in agency quality assurance activities and perform chart audits to enhance patient care quality.
Address Additional Documentation Requests/Denials as directed by regulatory affairs.
Train and mentor new Clinical Managers and provide ongoing support to care center teams.
Make employment recommendations to the Director of Operations based on performance evaluations.
Qualifications
Associate or Bachelor's degree in Nursing.
Current, unencumbered Registered Nurse license in the state(s) of assignment.
Minimum of 1 year of supervisory or management experience as a Registered Nurse in a healthcare setting.
Minimum of 1 year of RN experience in Home Health or Hospice.
Current CPR certification.
