Your Role As a Chronic Care Manager
As a Chronic Care Manager with us, you'll deliver compassionate care and patient education while leveraging the convenience of remote work. Engage with patients regularly, providing personalized support that fosters long-term relationships and empowers patients to manage their health effectively.
Responsibilities Include
Conduct regular check-ins with patients to discuss health updates and follow up on appointments.
Build rapport and trust with patients, understanding their unique needs and challenges.
Serve as a crucial liaison between patients and their care teams, ensuring seamless communication and coordination.
Utilize your nursing expertise to guide patients in understanding their conditions, advocating for their needs, and using technology for effective care management.
Qualifications
Compassion: You genuinely care about improving patient outcomes through empathetic care.
Licensing: Hold an active RN/LPN license in any U.S. state within the specified time zone. If required, we cover licensing fees for out-of-state patient care.
Experience: Minimum 4 years of nursing experience with chronically ill patients, ideally in home health or primary care settings. Proficiency in Electronic Medical Records (EMR) is preferred.
Skills: Demonstrate strong time management, communication, and tech-savvy capabilities essential for remote healthcare delivery.
