JOB SUMMARY: The Hospice Chaplain is responsible for the provision of spiritual care services to patients and families/caregivers of Bristol Hospice LLC, either directly or through coordination of care with other spiritual counselors.
Required Education & Experience
Must be a graduate of an accredited seminary or school of theology, BD, M.Div. or equivalent theological degree
Must have experience working with death and dying, individuals/family/caregiver
Hospice experience preferred

Must possess the ability to work as member of interdisciplinary group
Must demonstrate comfort in an interfaith setting
Must possess the ability to accept different lifestyles, cultures, beliefs and values
Must possess the ability to network with community, clergy and congregations
Must demonstrate knowledge of and commitment to hospice philosophy of care

Essential Job Functions
Assure spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner
Facilitate the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs
Provide direct spiritual care to patients and families/caregivers
Serve as liaison and support to community chaplains and spiritual counselors
Maintain records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision
Document direct services and ongoing communication with community chaplains and spiritual counselors
Attend patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group
Provide consultation, education and support to interdisciplinary group on spiritual care
Recruit community chaplains, spiritual counselors and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care
Provide for funeral or memorial services for patients as requested.
Plan periodic memorial services to meet the needs of personnel, volunteers and community clergy/spiritual counselors working with the hospice team
Assist in supervision of spiritual care volunteers when assigned to patients/families/caregivers
Actively participate in quality assessment performance improvement teams and activities
Accept and perform other related duties and responsibilities, as required

Must comply with company policies, procedures and processes in addition to Hospice and Homecare rules and regulations
Must be dependable and flexible in work schedule

Must have a safe and functional automobile, valid drivers’ license and full automobile coverage
Must be willing to submit to a criminal background check prior to hire and annually thereafter
PHYSICAL ENVIRONMENT:
May be required to stand and sit for long periods
Must be able to hear and see with or without aid
Must be able to read and follow instructions as required
Must be willing to participate in a drug/alcohol screen prior to hire, if involved in a work-related automobile accident and if reasonable suspicion is reported.
