As a Medical Assistant - Healthguide, you will play a crucial role in enhancing the connection between patients and their primary care physicians. You'll be part of a supportive clinical team dedicated to improving patient health and well-being.
What You'll Be Doing
Working primarily remotely, you'll collaborate closely with patients' medical practice staff and our Case Management team to support a targeted patient population.
Independently managing patient care while serving as an integral part of the medical practice team.
Assisting patients in achieving better health outcomes by addressing their concerns, coordinating care plans, and ensuring seamless communication with medical providers.
Documenting patients' medical histories and issues accurately in electronic health records (EHR).
Facilitating referrals to specialty care and community-based organizations as needed.
Building and maintaining professional relationships with assigned medical practices and within our organization.
Meeting Key Performance Indicators (KPIs) to ensure positive patient outcomes and reduce medical costs.
Using effective communication methods (calls, texts, portals, emails) to engage with high-risk patients.
Escalating medical issues promptly to the appropriate healthcare professionals or leadership.
Occasional Travel To Primary Care Physician Offices As Required.
Adhering to all security policies to safeguard Protected Health Information (PHI) and intellectual properties.
What You'll Need To Have
Certification as a Medical Assistant (preferred) or Certified Community Health Assistant, with at least 2 years of relevant experience.
Passion for patient coordination and serving individuals with complex medical and social needs.
Strong empathy and proficiency in motivational interviewing and behavioral change management.
Exceptional communication skills and proficiency in documenting patient interactions in real-time using software.
Knowledge of medical terminology and primary care physician practice operations.
Understanding of social determinants of health.
Bilingual proficiency in English and Spanish strongly desired.
Familiarity with electronic health records (EHRs) and care management databases.
Ability to prioritize tasks effectively while maintaining attention to detail.
Comfortable working independently within a remote work environment.
Familiarity with EHR systems such as EPIC, Athena, or Cerner is advantageous.
