N Street Village also provides affordable rental housing for low and moderate-income individuals and families.
Position Summary
The Case Manager plays a pivotal role in coordinating and facilitating comprehensive care for clients. You will assess client needs, develop individualized care plans, and collaborate with multidisciplinary teams to ensure clients receive appropriate services. Your responsibilities will include advocating for clients, monitoring progress, and connecting them with community resources. Strong communication, organizational, and problem-solving skills are essential for success in this role.
Specific Job Duties/Responsibilities
Collaborate with clients to develop individual service plans within 30 days of placement and then every six months thereafter, or when a goal has been achieved
Coordinate with the multidisciplinary team about appropriate referrals or solutions for challenges related to housing, financial concerns, employment, recovery, mental health
Facilitate regular groups for clients on life skills to include topics such as, communication, problem solving, decision making, time management, cooking and nutrition, emotional intelligence, and conflict resolution
Meet with the client bi-weekly for face-to-face case management engagement, or the required number of interactions based on contractual requirements
Maintain confidential files, records, and data on each resident
Create and maintain case notes, including but not limited to legal documentation, assessments, medical reports, referrals, educational, service updates, etc
Enter information into HMIS, HTH and/or NSV CRM in a timely manner, adherent to contract requirements and best practice standards
Participate in organization-wide meetings and committees, departmental meetings, team meetings and training as assigned
Provide assistance to clients for observed concerns regarding sobriety, mental health, physical health, grief, etc
Present psychosocial needs, strengths, and concerns of clients during supervision, interdisciplinary meetings, and care coordination meetings
Report and document all incidents, allegations, grievances, and sensitive situations to the Program Manager immediately
Other duties as assigned
Education And Experience Qualifications
A combination of education and experience to include at least 5 years of relevant experience with a focus on one of the following areas: mental health, physical health, addiction, aging, trauma
High school diploma, bachelor’s degree preferred in Human Services Discipline to include: Psychology, Social Work, Mental Health, Criminal Justice, or a related social service discipline and 2 years’ experience providing case management services
Bachelor’s degree in social work must be licensed at the LSWA level
Knowledge of DC social services resources and compliance requirements
Skills And Competencies Required
Strong cultural competence, including excellent interactions and communication ability with a diverse population
Mature abilities in developing interpersonal relations, communication, problem-solving, conflict resolution
Ability to maintain confidentiality and healthy professional boundaries
Ability to multi-task with strong organizational skills and attention to detail
Knowledge of Microsoft Word and Excel, and the ability to learn a database system
Knowledge of HMIS
Ability to work independently
