We are seeking a dedicated Care Coordination Navigator to join our healthcare team. This role plays a critical part in guiding patients through the complexities of the healthcare system, ensuring seamless transitions between care settings, and promoting comprehensive, patient-centered care. The ideal candidate will possess strong interpersonal skills, organizational abilities, and a deep commitment to improving patient outcomes through effective care coordination.

Your Impact

  • Serve as a primary point of contact for patients, caregivers, and healthcare providers to facilitate smooth transitions of care
  • Conduct initial assessments to identify patient needs, goals, and barriers to care
  • Develop personalized care plans in collaboration with multidisciplinary healthcare teams and patients
  • Coordinate appointments, referrals, and diagnostic tests to ensure timely access to healthcare services
  • Advocate for patients to ensure they receive necessary treatments, medications, and supportive services
  • Educate patients and caregivers about their health conditions, treatment plans, and self-management strategies
  • Monitor patient progress and outcomes, adjusting care plans as needed to optimize health outcomes
  • Document all patient interactions, care plans, and interventions accurately in electronic health records (EHR) systems
  • Collaborate with insurance providers and community resources to address social determinants of health and facilitate access to supportive services
  • Participate in care conferences and meetings to discuss patient progress, goals, and discharge planning
  • Perform other duties as assigned by management needed to help drive to our Vision, fulfill our Mission, and abide by our Organization’s Values


Your Credentials

  • Bachelor’s degree in nursing, Social Work, Healthcare Administration, or related field; master’s degree preferred
  • Proven experience in care coordination, case management, or healthcare navigation roles
  • Knowledge of healthcare systems, medical terminology, and patient care processes
  • Strong interpersonal skills with the ability to build rapport and communicate effectively with diverse stakeholders
  • Excellent organizational skills and attention to detail
  • Ability to work both independently and collaboratively within a multidisciplinary team
  • Proficiency in using electronic health records (EHR) and other healthcare software applications
  • Understanding of healthcare regulations, policies, and confidentiality guidelines
