We are seeking a dedicated Care Coordination Navigator to join our healthcare team. This role plays a critical part in guiding patients through the complexities of the healthcare system, ensuring seamless transitions between care settings, and promoting comprehensive, patient-centered care. The ideal candidate will possess strong interpersonal skills, organizational abilities, and a deep commitment to improving patient outcomes through effective care coordination.
Your Impact
Serve as a primary point of contact for patients, caregivers, and healthcare providers to facilitate smooth transitions of care
Conduct initial assessments to identify patient needs, goals, and barriers to care
Develop personalized care plans in collaboration with multidisciplinary healthcare teams and patients
Coordinate appointments, referrals, and diagnostic tests to ensure timely access to healthcare services
Advocate for patients to ensure they receive necessary treatments, medications, and supportive services
Educate patients and caregivers about their health conditions, treatment plans, and self-management strategies
Monitor patient progress and outcomes, adjusting care plans as needed to optimize health outcomes
Document all patient interactions, care plans, and interventions accurately in electronic health records (EHR) systems
Collaborate with insurance providers and community resources to address social determinants of health and facilitate access to supportive services
Participate in care conferences and meetings to discuss patient progress, goals, and discharge planning
Perform other duties as assigned by management needed to help drive to our Vision, fulfill our Mission, and abide by our Organization’s Values
Your Credentials
Bachelor’s degree in nursing, Social Work, Healthcare Administration, or related field; master’s degree preferred
Proven experience in care coordination, case management, or healthcare navigation roles
Knowledge of healthcare systems, medical terminology, and patient care processes
Strong interpersonal skills with the ability to build rapport and communicate effectively with diverse stakeholders
Excellent organizational skills and attention to detail
Ability to work both independently and collaboratively within a multidisciplinary team
Proficiency in using electronic health records (EHR) and other healthcare software applications
Understanding of healthcare regulations, policies, and confidentiality guidelines
