Peoples Health Care Connection is seeking a therapist who will be responsible for providing individual, group, marital and family therapy to clients in the community. This is an excellent position for someone who wants flexibility in their schedule. This is not a remote position.
General Responsibilities
Contributes to Peoples Health Care Connection mission, vision, and program philosophies and maintains a professional behavior with both internal and external consumers
Adheres to all current federal & state laws, as well as Peoples Health Care Connection Policies and Procedures, including safeguarding of confidential healthcare information and compliance with the Health Insurance Portability &
Maintains all required and appropriate documentation for clinical records and progress notes for assigned cases in accordance with agency policies and procedures.
Conducts and documents screening and assessment of adults, adolescents and/or children in order to elicit information concerning presenting problem, demographic information, and medical history in accordance with agency specifications.
Completes and documents psychosocial histories and assessments for clients in accordance with agency specifications.
Complete and updates individualized treatment plans that are client centered, strength based, developing goals and objectives that are measurable.
Coordinate client care with the treatment team.
Knowledge of addictions, recovery, mental health and other co-occurring disorders. Excellent interpersonal skills and the ability to maintain a positive collaborative relationship.
Experience in crisis intervention and conflict resolution skills.
Experience working with an ethically, culturally and racially diverse client population; ability to work harmoniously with a wide diversity of individuals.
Represents the center in a professional manner within the community, including being a liaison with other behavioral health facilities, medical health facilities, area schools and community agencies.

Demonstrates an awareness, appreciation and respect for diverse cultures and individual differences of clients and fellow employees; identifies culturally relevant issues and implements communications in a manner appropriate to them.
Maintains all training requirements for licensure
Attends staff/agency meetings as required
Reports to the Clinical Director as necessary regarding support services, personnel issues, etc.
Completes other duties as assigned
Accountability Act (HIPAA)
Provides direct client services in the community and outpatient setting using evidenced based practices.
Qualifications
A Master’s degree from an accredited university or college with a major in the field of counseling, social work, psychology, or a related human service field, and two (2) years professional experience providing services to adults, adolescents and/or children with serious mental illness and/or substance abuse/addiction.
Must be a MSW, LMSW, LAC, LPC or LCSW
Undergo and pass criminal background check
Valid Class One Fingerprint Clearance Card
Current CPR and First Aid Certification
Negative TB Test (current within 12 months prior to employment)
Covid-19 Vaccination
Current valid Arizona driver’s license
Proof of valid/current auto insurance
Three references
Copy of college transcripts/college diploma
